Once printed & completed, please sign and date, and then fax or mail your child's application to our office. (Only fax your child's application if you plan to pay via credit card.) Please note that every application must be accompanied by a deposit.

Let's Go Green and Begin Paperless Communication!
As of February 26th the Confirmation letters (usually sent by mail) will be sent to families by E-mail! In an effort to conserve the environment we felt this is yet another step in this important cause and it is our goal to eliminate all un-necessary paper waste in the future.
